Passport Application

Correct version of the U.S. passport application should be used failing which the passport will not be issued.

You can use DS-11 passport application form:
If you are applying for a passport for the first time
If you are under the age of 16
If you have a passport which was issued to you 15 years ago
If your passport is stolen or lost recently

You are advised to read the instructions before filling the passport application form. Follow the instructions in page 1 and 2; submit valid documents to support page 3 and 4.

A previously existing passport can be renewed with DS-82 passport application form at the Regional Passport Agency.
You may be required to submit your old passport for renewal
You must submit your passport which was issued to you 15 years before.
You must submit your passport which was issued when you were under 16 years of age.

There are different passport application forms:
If you have not had a previous U.S. passport you must use the form DS-11.
If you have already had a U.S. passport and your intention is renewal of the passport then use form DS-82.
If you intend to change or have correction of the U.S. passport then you may need to use DS-5504 which is the reapplication form for U.S. passports.
If you intend to have more pages to your U.S. passport DS-4085 should be used.
If you intend to report lost or stolen passport DS-64 statement of lost passport should be submitted along with DS-11 application.
If you intend to apply on behalf of a minor child you should use DS-3053 consent for passport issuance for the minor along with the D-11 application form.

Every passport application whether it is DS-11 or DS-82 consists of four pages. Of which 2 pages details on the application; the first two pages are not meant for submission, they are user instructions. The third page has columns to be completed by the applicant. The fourth page in the application does not have any column to be completed by the applicant, but it has got legal statements that are meant for official purposes and is required as a component of the application form.

The pages 3 and 4 should be correctly filled in according to the instructions given in pages 1 and 2 and should be submitted with valid documents are required by the authority accepting the application forms.
